Side Venom Lacrosse Club Mission Statement
West Side Venom Lacrosse Club has the mission to facilitate lacrosse play through
the CJLA/CGLA venue, for children from 2nd grade through 8th (boys and girls) living
in the West Metro Denver area. Our mission includes teaching lacrosse
fundamentals, good sportsmanship, and the essentials of team play, including,
but not limited to cooperative effort. Each player will be encouraged to develop a
strong moral ethic, both on the field and through life, with respect for others' wellbeing,
property and safety, while pursuing the best for their team. As volunteers, we strive to provide
a positive life experience for developing a lifelong enjoyment of lacrosse.
Player’s Code of Conduct
I have read the West Side Venom Lacrosse Club (WS Venom) Mission Statement and agree with its intent. As a player for WS Venom, I accept the responsibility of doing my best both on and off the field to improve my lacrosse skills and improve myself as an athlete of high moral and ethical practice. Towards this goal, I accept the responsibility of being on time for practice and games, ready to play, listening to my coach and obeying the rules of the game. I agree not to play without my protective equipment (helmet, pads, etc.) or play with someone who is not properly protected. I understand that Lacrosse is a team sport and that I am but one member of a team who needs me to do my best for the team. As a player on a recreational team, I may expect to be played some, but not all, of the time. I understand that playing is a privilege and not a right, and may lose that privilege at any time for inappropriate behavior.
Parent’s Code of Conduct
I have read the WSLC Venom Lacrosse Club Mission Statement and agree with its intent.
As a parent of a for WSLC Venom, I accept the responsibility of doing my best to support
growth in my child's lacrosse skills and as an athlete of high moral and ethical practice.
Towards this goal, I will assist my child in being on time for ALL practices and games
(unless otherwise excused by coach), ready to play, and encourage him/her to listen to the coach and obey the rules of the game. I understand that Lacrosse is a team sport and that my child is but one member of a team who needs every player to do their best for the team. As a player on a competitive team, I understand my child may play at the coach's discretion and will support the coach's decision. I understand that the coach is a volunteer and that I should offer assistance before criticism. I understand that playing is a privilege and not a right, and that my child may lose that privilege at any time for inappropriate behavior. I will show respect for the coaches and referees and will display good sportsmanship at all times.
West Side Venom Lacrosse Waiver and Release of Liability and Photos
As the parent or legal guardian of my child, and by registering him/or her as a participant in the West Side Venom Lacrosse Club ("WS Venom") sports program, (and all related events and activities, to include practices, games, clinics, camps, tournaments and travel), I do hereby acknowledge, and agree that:
1. The risk of injury from the activities involved in this program is significant. I have independently reviewed and evaluated the risks and determined that my minor child may participate in the program, and I assume all responsibility, with my full knowledge and acceptance of the risk.
2. I, for myself, and on behalf of my child, my child's successors, heirs, assigns, and personal representatives, agree that WS Venom , including all participants, officials, coaches, agents, officers, employees, sponsoring individuals or agencies, advertisers, owners or lessors of premises used in conducting the program, are hereby released from any and all liability and claims for any injuries, disability, death, or loss or damage to person or property, incident to my minor child's participation or involvement in the West Side sports programs, even if caused by the negligence of WS Venom or its agents.
3. My minor child will comply with the stated and customary rules and regulations for participation in the program.
4. While not required, the registrant is encouraged to seek approval of their physician prior to commencing the program.
5. Any photos taken of child during this program and all its related events may be used by WS Venom in promoting the club and the sport of lacrosse, either on its website or in printed material, distributed by WS Venom and its affiliated organizations.
